How to Apply


  1. Fill out the scholarship application. All items are required, if it doesn’t apply, enter N/A.
    • It is a PDF that you can type your information into, save and edit as needed.
    • You (& your parent/guardian if you’re younger than 18) need to sign & date the application.
  2. Complete your Academic Resume
  3. Complete your Activities Chart
    • Log into your CIS portfolio account at OregonCIS
    • Under the Education heading (top of the page) click on “Activities Chart.”
    • When a new page opens, scroll down to the bottom of the page and click on the “Create Draft Chart” button.
    • Once the document is open in Word, adjust the total hours for each activity.
    • If you have questions, talk to Brenda Richardson in the Counseling Center.
  4. Review the list of available scholarships
  5. Complete the scholarship selection form
    • Print 2 copies of the confirmation page once you submit the scholarship selection form and add it to your packet.
    • If you select scholarships with additional criteria, make sure this information gets turned into Counseling with your application.
    • Write and submit a typed essay, approximately 500 words comprised of an introduction, body paragraphs and conclusion. The body paragraphs should address each of the four following questions:
      1. What are your specific educational plans and career goals and why? What inspires you to achieve them?
      1. What have you done for your family or community that you care about the most and why?
      1. Describe a personal accomplishment and the strengths and skills you used to achieve it.
      1. Describe a significant change or experience that has occurred in your life. How did you respond and what did you learn about yourself?
  6. 2 Teacher recommendation letters
    • Select two Teachers and/or one Teacher and one Staff member to write you a letter of recommendation.
    • Make sure that the Teacher or Staff person you ask knows you well.
    • Print our your activities chart or a resume. Give it to the Teacher or Staff person to assist them in writing you a letter.
    • Give each Teacher and/or Staff person a reasonable amount of time to complete your request. Politely, check back with each Teacher or Staff person to make sure it is completed.
    • Plan ahead and don’t wait until the last minute to make your requests.
  7. 2 Community recommendation letters
    • Print the instructions (PDF) that you need to give to the person you are asking for a recommendation.
      You are encouraged to use the senior College Prep Class credit required letters of recommendation.
  8. Transcript – Classes through the 2nd quarter should be included. Your transcript release is covered with the signatures on the scholarship application.

Bring your scholarship packet to Brenda Richardson in the Counseling Center by Tuesday, January 9th, 2018 (essays must be completed). Late applications will not be accepted.

Essays are due Friday, December 8th for review, help, and suggestions.

Packet check by Mr. Potts is Friday, December 15th. Your scholarship packet needs to contains in this order:

  • Scholarship Check List
  • Completed, signed and dated scholarship application
  • Transcript
  • Academic Resume
  • CIS Activities Chart
  • 500-word Goal Statement
  • 4 letters of recommendation (2 school and 2 community)
  • Additional scholarship criteria (if needed). Label each page with your name and the name of the scholarship.

Keep a personal copy of your application and all materials in a hard copy file at home. Keep your electronic file materials well organized on your own computer and have backup on a flash drive for easy access in case you need to revise, edit or print items quickly to meet other important scholarship deadlines.
